Summary: Problem upgrading from t64 5.1 to t64 5.1A

Ok:

Lesson learned: NEVER try to install an upgrade from a non-mount
point. You will hose you're cluster. If I had created a tmp ADVFS file
system and copied the contents of the CD onto the new filesystem it should
work. However, I don't think I'll try upgrading from anything other than
the CDs again.

Upgrade path from 5.1 to 5.1BPK should go something like
this: Rolling upgrade from 5.1 to 5.1A - this means the full
procedure. Then another rolling upgrade from 5.1A to 5.1B (but you can put
down PK1 during this).

Thanks for all the help everyone.
