Failed Mirror Disk

Hello everyone,
        Not sure how to ask this question. I basically have a failed disk in a
SUN Enterprise 420 and I am unsure which disk it is. I cannot figure out
which disk is on what controller and maybe someone with a similar
machine could tell me. It is running Solaris 8 and disksuite. I need to
know if anyone can tell me which disk it is because the format command
and the disk location of course do not match up.

I think its disk 11, but not sure. Can anyone tell me for certain?
