Corrupt label - wrong magic number!!

Hi Sunmanagers,
                        Wish u all a very happy new year. i have a disk
related issue on one of our production servers (it is a SPARC Server 1000E).
/var/adm/messages shows that a disk (c1t5d1) has a Command read error. it
also reports a "Corrupt label - wrong magic number" error. analyze>read
shows 0 errors; cannot run analyze>test or refresh, it reports :- "Cannot do
analysis on a mounted partition"*

*ran *# *prtvtoc /dev/rdsk/c1t5d1s2*** *it returns with the
following message:-

***prtvtoc: /dev/rdsk/c1t5d1s2: Unable to read Disk geometry*

However the partition table of the disk is visible from format>partition>p

*when trying to label the disk with format> label it says:- *Cannot label
disk while it has mounted partitions.*

How do i fix this problem, has the disk gone bad, do i need to replace it?
Kindly advise on this.
