df -k reports wrong disc after boot failure

Hello Gurus!

I am experiencing a problem with a Sun Ultra 10 running Solaris 2.6. Upon
booting, I get an UNEXPECTED INCONSISTENCY error from fsck when it tries to
check /. Every subsequent filesystem check also fails for the same reason.
This causes the boot process to fail.

When I login in single-user mode, df -k seems to think that every mount is
using /dev/dsk/c1t0d0s?. instead of /dev/dsk/c0t0d0s?. Additionally, format
gives a 'No Disks Found!' error, even though probe-ide correctly identifies
the disc (a single 18.64GB IDE) at the OpenBoot prompt. /etc/vfstab, /dev
and /devices are all set up correctly.

Bizarrely, if I boot from CD-ROM, I can fsck the disc and mount all of the
partitions without any trouble at all, and format detects the disc properly
too.

This problem is preventing a system update from going out so any suggestions
as to what may be causing it, and how to resolve it would be greatly
appreciated.
