btw. I hate to muddle this up, but in my experience these other requirements
must be met:
unmirrored rootvg
lppchk -vlc/grpck -n ALL must be successfull
/etc/group must not have any lines over 2048 char(i.e. must be able to vi
/etc/group)
cdrom f/w can be problematic - upgrade first
hd5 must be >8MB
hd5 must be on outeredge and 1st sectors(mklv -t boot -ae -y lv5 rootvg 2
hdisk#)

well, at one of my system it is
root@0-tvgicsc1 160# lslv -m hd5
hd5:N/A
LP PP1 PV1 PP2 PV2 PP3 PV3
0001 0110 hdisk0

so pp 110 and yours is 142. should be fine, do you still have the same
problem?

This is no good: it must be contiguous physical partitions, starting from 1.
