well, at one of my system it is
root@0-tvgicsc1 160# lslv -m hd5
hd5:N/A
LP PP1 PV1 PP2 PV2 PP3 PV3
0001 0110 hdisk0
so pp 110 and yours is 142. should be fine, do you still have the same problem?

This is no good: it must be contiguous physical partitions, starting from 1.
