SUMMARY: lsm vminor error

Problem:

TRU64 5.1a been running fine for a awhile.

received this error on re-boot

dsfmgr: hwc_services failed to find device node (201300B6)

after the error booted into single user mode. Cannot mount /usr,var.
Receive error "lsm:volio: illegal vminor encountered:
/dev/vol/rootdg/usrvol"

Also cannot mount many of my HSZ50/70 devices even though the
controllers report no errors and when I do a "show dev" at the console
the devices can be seen.

Resolution:

an error communicating with an HSZ50 started the whole problem even
though no LSM data was stored on those devices. Device names (dsk) were
corrupted. Resolution: ran mountroot, bcheckrc, ran dsfmgr -e to
correct device names.
