Ultra5 panic and endless reboot

Hello,
   I've got an Ultra5 that panics on boot, saying it can not read
name_to_major. Attempting to boot from the network eventially gets to the
message telling me that devices are being configured, and the spinner
(stylus)
never stops (I let it go for 20 minutes thus far). The exact messages
when attempting to boot from the disk in the system are as follows:

   I ran test-all and everything passed successfully (except for the
display, as I was working from a serial console).

   Are there any suggestions for further diagnosing the problem?
