Multiple x6727a Cards Issue

I installed two x6727a dual fiber HBAs in a Sun E220R in the 33 MHz
slots running Solaris 9. These are two quad fiber dual channel cards
totaling four channels. The system discovers them and assigns cxtxdxsx
device names to them although the target numbers are not consecutive
past 15. They are connected to two a5200's running in split loop mode
and the four HBA channels are connected to them.

The problem is the SUNWqlc driver is not attaching to all of them.
Prtdiag shows one correctly discovered but the other incorrectly.

The format command shows separate driver device paths to the same type
of hardware in the middle of the disks: (scsi vs. SUNW,qlc)

Either card works with the correct driver in a single card installation
after a clean jumpstart. I tried moving them around in and out and even
using the single 66 MHz slot and regardless what configuration is used
the result is the same. Running the format command shows that half the
disks, corespoding to the prtdiag command, are not attached to a QLC
managed device.

Is there a way to resolve this? Does the qlc.conf file need something
added to it to use that driver with the second card? This seems like a
simple problem but I have been unable to find a clearly documented
solution.
