Hi gurus.
I finally got my system back to it's normal, happy self. Thanks to the following individuals for their time and energy:

Everybody gave me some really good suggestions. These included: run truss on the process to see where it's hanging (this showed where it was hanging but didn't provide any explanation as to why it failed); editing the /etc/lvm/md.tab and then running the various commands; tryu to use other mirrors, like d1; uninstall and reinstall the software; and Sam and Francisco game the answer I finally used: reboot. That cleared everything else so I was able to create the primary mirror.
