devfsadm: driver failed to attach: st

Hi all,

I have a perplexing problem:

In a nutshell, /dev/rmt/0cbn disappears on an adhoc basis.

Details
-------

We have HP-Ultrium1 2/20 zoned to a set of hosts in a SAN fabric.

Typically all hosts in the zone can access the HP-Ultrium1 2/20 via FC. However, on
an adhoc basis the device node on one of our solaris boxes 'disappears'.

e.g

# ls -l /dev/rmt/0cbn
lrwxrwxrwx 1 root root 72 Aug 30 10:24 /dev/rmt/0cbn -> ../../devices/pci@1e,600000/SUNW,qlc@3/fp@0,0/st@w100000e00221f9b9,2:cbn

# ls -l /devices/pci@1e,600000/SUNW,qlc@3/fp@0,0/st@w100000e00221f9b9,2:cbn
/devices/pci@1e,600000/SUNW,qlc@3/fp@0,0/st@w100000e00221f9b9,2:cbn: No such file or directory

# devfsadm

# rem_drv st

# add_drv -v st
exit status = 0
devfsadm: driver failed to attach: st
exit status = 11
Warning: Driver (st) successfully added to system but failed to attach
Driver (st) installed.

If we reboot the box the device node is re-created and all works fine.

The box in question is: Solaris 9 4/04 s9s_u6wos_08a SPARC

The HBA is a SunFC Qlogic FCA v20040305-1.38

Can anyone suggest other methods to debug/diagnose this problem ?
