Missing A1000 module after creation of new Fujitsu LUN

Hello Guru's,

I hope you can help me, I'm on a live system and have a long night ahead
of me now.

Problem:
I administrate two Sun E450's Solaris 5.7 connected to two StorEdge
A1000's (dual host connection). I'm using Raid Manager 6.22.01.14 with
Veritas VM 3.0.4

Today I added two Fujitsu 72gb drives into both A1000. This went very
smoothly running healthck and then doing the Sun advised fail and revive
technique....all OK. (Note I am running RM commands and GUI from one
server as advised)

Now I decided to create my lun, the design is to use LUN 5 on either
module. Each LUN will consist of two Fujitsu MAP3735N SUN72gb drives
I begin to create my LUN 5 on module c3t0d0, when RM finished formating
its status is optimal.

Now I decide to check my console I see the following error message

Now I'm worried so I run the format command

My module/array c3t0d0 has disappeared !!!!!!! What can I do !!!

This is the output from the other server, note it cannot see the newly
created LUN either but at least array c2t0d0 has not disappeared.
