Help ... System won't boot after mirroring /usr

Hi All,

I installed DiskSuite 4.2.1 on top of Solaris 8 and jsut finished mirroring
the /usr filesystem after using format to configure the partitions. These
are the commands I used for mirroring /usr -

Created state database replicas -

bash-2.03# metadb -a -f -c 2 /dev/dsk/c0t0d0s6
bash-2.03# metadb -a -c 2 /dev/dsk/c0t1d0s6

Created submirrors and mirror -

bash-2.03# metainit -f d12 1 1 /dev/dsk/c0t0d0s3
d12: Concat/Stripe is setup
bash-2.03# metainit -f d22 1 1 /dev/dsk/c0t1d0s3
d22: Concat/Stripe is setup
bash-2.03# metainit d2 -m d12
d2: Mirror is setup

Edited vfstab with this entry -

/dev/md/dsk/d2 /dev/md/rdsk/d2 /usr ufs 1 no -

Attached submirror -

# metattach d2 d22

This worked fine and it resynced to an okay state. Was happy at this point.

Then I decided to reboot, and it all went pear shaped. This is what I got
on reboot -

WARNING - /usr/sbin/fsck not found. Most likely the
mount of /usr failed or the /usr filesystem is badly
damaged. The system is being halted. Either reinstall
the system or boot with the -b option in an attempt
to recover.

syncing file systems... done
Program terminated
{1} ok

Help .. Have no idea what has happened. This happened once already and I
rebuilt the server and started again cause I couldn't fix /usr and get the
system to reboot !!!!!!!

Any ideas ???
