The biggest difference between External/permiter testing and internal testing is the level of access you have. Internally, it's assumed that you will have fewer "security" devices inbetween your testing system and the systems you're evaluating. That usually also means a far greater number of hosts to evaluate. You could use the OSSTMM for the internal testing the same as you would for external.

If you're local/internal, it may also be assumed that you're going to have a login to the systems being tested, and you'll be using that login to evaluate patch sets, file permissions, etc. That sort of testing is not in the OSSTMM yet, but may be added in the 3.x series.

Just keep in mind that the OSSTMM is a baseline. It's something to help a tester deliver consistent and thorough results, but shouldn't get in the way of your creativity while testing.
