I do them myself.

No. I've never experienced a problem during a firmware update.

Just be careful. I always make sure to check the instructions
each time I download and install a new level: I have seen them
change between levels and of course they're different between
models, so it's dangerous to rely on your memory, or an old
document. The up-to-date instructions will also include
check-sum information.
We've been doing a lot of firmware upgrades recently: Silver and
WinterhawkII nodes, plus M80/6M1 and 6H1 SP Attached servers.
We did have a problem with the 6H1 code: following an upgrade to
CM_MM_030829 migration from 4.3.3 to 5.1 failed, so we've stuck
with 021213.
I haven't had any other problems, though.
