I've never done this myself - I always just ftp the firmware to the target
machine - but my reading of the instructions is that it's just a paper
label.

Look near the top of 5.1.2.1:
"Two 2MB (HD) new or freshly formatted DOS diskettes are required."

If it wanted an unformatted diskette, or one formatted in some other way it
would say.
