Can't create optional boot device after upgrade

Hello,
I have a situation that I need solved. I have talked to Prophet 21 and
their expert is stumped by it.

RS6000 7025-F40
AIX 4.3.3 was 4.2.1
FC 6025 PCI Dual Channel Ultra2 SCSI Adapter
6 Drives

Last week I tried an upgrade from 4.2.1 to 4.3.3 ML 08

1st Step Migration to 4.3.1 failed with auto script due to this
driver. devices.pci.00101010 I think. I went ahead and tried to
install it manually. It seemed to go fine.

2nd Step Update 4.3.x to 4.3.3 failed also due to the following drivers
    bos.loc.utf.ZH_CN
    devices.pci.00100c00.rte
(The first one I had instructions in case it failed to install it
manually so I did those. The 2nd one I modified the first instruction to
the name and it looked like it worked fine. I got the ok afterwards on
both)

3rd Step Maintenece Level 08 failed on the following drivers
    devices.isa.sio_baud.rte 4.3.2.1 : 4.3.0.0 -- AIX 4330-08 Update
    devices.pci.00100b00.diag: 4.3.3.25 : 4.2.1.0 -- AIX 4330-08 Update

    devices.pci.00100c00.diag :
(As above I applied the drivers manually and seemed to work.)

Our Hard drives are mirrored. Disk0 is primary system drive. Disk3 is
the mirror copy of the system drive.

I tried to mirror the rootvg Disk3 back to the Disk0 and it failed.
