FYI - we might want to look into this APAR, if it is not applied.

Machines Affected: 7040 Systems
Title: System Boot failures on p670 system or p690 system

Description: You may receive customer complaints that their system
will not boot or boots repeatedly after either installing new device
drivers or addition of a new device to the system. There are no particular
LED hangs or SRN/error code hangs associated as it is dependent upon which
device is involved. This problem can appear to be a hardware failure when
it is not and is extremely difficult to diagnose.

Action: Have the customer/sys admin verify/install the AIX component,
"devices.chrp.base.rte 5.1.0.26", which has "APAR IY31424" included before
upgrading any firmware, device drivers, or adding new devices to the
system. Failure to perform this step can result in failure of system boot
and repeated system boot until failure.

Workaround: If your customer has already reached this stage of
failure, the customer should boot the system into "Maintenance Mode" and
apply this APAR. If further assistance is needed, please contact "Software
Support".
