ES40 - KZPCC-CE FAILED DRIVE RAID QUESTION

Hello,
I have an ES40 running TRU64 v5.1A. The controller card is a KZPCC-CE 3 channel with 12 18.2GB drives in a RAID5 and RAID1 configuration.

Two drives lit up as failed at the same time. I started SMOR and the one drive (2,5) was marked as 'Missing Component' and the RAID5 had a status of DEGRADED. In this state I could not mount the cdrom device and on my backups the /dev directory was empty. All other drives were marked as Optimal but the drive (1,4) not longer appeared in the raid configuration.

I replaced the (2,5) drive with an identical drive and the rebuild completed successfully and I can mount the cdrom device. I started SMOR again and all drives were optimal but the drive (1,4) was still not in the configuration and it still indicator light is still red.

My RAID1 consists of drives (1,0) and (2,0)
My RAID5 consists of drive (1,1) (1,2) (1,3) (1,5) and (2,1) (2,2) (2,3) (2,4) (2,5)

The drive (1,4) was originally part of the RAID5 configuration.

My question is: Why does this drive no longer show up in the RAID5 configuration. I am hesitant to remove the drive and replace it because I am not sure of the implications of this scenario.
