SUMMARY: Trouble installing new disk in 5.1A : cannot create device files

Managers,

First of all, many thanks to those of you who wrote me with suggestions on
how to deal with this problem (Raul and Tommy esp.). I got a lot of
valuable information on how to deal with devices under 5.1A.

Problem:

My problem was stated originally as a question on how to get 5.1A to create
device files for the new LVD disk I added to the system. The disk was added
to a new controller (!) and although the disk was seen at the firmware
level, I couldn't get the system to see it when booted and so I couldn't
proceed to label the disk, etc.

Solution:

As unsatisfying as this is, it turns out that the brand new 3X-KZPCA-AA LVD
controller was faulty. More info following...

No matter what I tried I could never get the device to be fully recognized
and even though the system once created the /dev/disk/dsk# files, I could
not use them. The best I could get was this:

I still could not label the disk. However, someone suggested I check to see
if any errors were logged. Since I was only booting into single-user mode,
nothing was logged to /usr/adm/messages, but when I booted into multi-user
mode I found:

I Google'd this error and found reference to a problem with the controller
(3X-KZPCA-AA -- LVD SCSI adaptor)

This is not satisfying at all, but it turns out that the controller was
faulty. I replaced it and everything works perfectly.
