SUMMARY: Strange SCSI error

Thanks to Paul Thompson and Dr. Blinn for their help.

As it turns out, this is a peculiarity in the way that the list of
device paths is implemented. To paraphrase, the entry for the first path
ever seen is part of the master data structure for the device. If you move
the device, that path entry can never be deleted.

The answer was to delete all of the hardware entries and let them be
rescanned. Fortunately, this wasn't necessary for the root drive (perhaps
because it hadn't been moved), so the solution was:

        Unmount all drives except root.
        Comment entries out of fstab.
        Delete entries from hwmgr.
        Reboot.

This left me with device names up in the dsk40+ range, so I used
dsfmgr -m to exchange device file names, and modified the fstab to mount the
new names. I now have dsk1 to dsk7 for my disk device names, and haven't
seen any errors since the second reboot.
