SHC software mess!!

I recently attempted to install the "System Healthcheck" application. As
this has been certified for the DS20 machine and Tru64 v5.1 (as per the User
Guide), I installed on our DS20 m/c. However, during the installation the
system crashed. On restarting it, I noticed all applications except the SHC
to be working fine. Hence, I decided to delete this from the system.
However, on deleting it, the m/c crashed again! I have since not been able
to delete it and the "setld -i" shows the SHC status as below:

I would like remove this software from the m/c but it looks like only a
manual procedure can now get rid of it. Can someone help me on how to remove
this application manually so that it is neither reflected in the m/c nor in
the file system? Would appreciate a prompt reply. Thanks.
