ES40 and Mylex controller

Hello Managers ,
                   I have an ES40 system with a mylex controller
we had one of our disk fail on this contoller .
Now I have replaced the failed disk and I would like to set the same
Online.

Can someone point me in the right direction of how do I go about
enabling the disk .

I kind find any documentation pertaining to this .

This is the message I get when the system boots up

The system is ES40 running Tru64 4.0F pak 7 .
