Upgrading from 4.0c/d/f to 5.1.

I was wondering about the steps involved in upgrading from 4.0[cdf] to
5.1x?

Currently, at different sites, we have machines with all the of the above
OS'es. I seem to remember that we can't go directly from certain versions
of 4.0x to 5.0, and that we HAD to go to 5.0 before going to 5.1.
