Lockout after 5 failed login attempts does not function properly

Operating a stand alone 21164 LX workstation running Tru64 5.1 with
Enhanced Security. Account lockout set to 5 failed login attempts.

Current Actual Behavior: Attempt to login with incorrect password more than
5 times and system continues to let one try passwords. Then when correct
password is used, notification is provided that the account is locked and
to contact the system administrator. Then when logging in as Root, the
account is found to be disabled and not locked at all.

Desired Behavior: Attempt to login with incorrect password more than 5
times and system Prohibits any further attempts using that user id until
the SysAdmin goes in and finds the account locked and unlocks it.

I need to know how I can configure the system to behave in the desired manner.
