Thanks to all who replied to my original question, especial Dr Thomas Blinn,
Phil Baldwin, Pete Sherwood, Jenny Bulter and everyone else.

It turned out the be what Dr Blinn said - it a problem with the tape drive.
I called HP and replaced it and all is good.
Attached is Dr Blinn's reply and most of my original question.

Well, your device is reporting an unrecoverable hardware error.

I don't know that it has an error, but that's what it reports. It
is right there in plain English in the output from the "mt status"
command.

Now, it's possible that patch kit 5 did something horrible to one
of your system files that influences magtape handling; check the
ddr.dbase and ddr.db files; there may be a patch that modified
one or the other or both (it should touch both) that just might
have broken the access to the drive. But, "mt status" says that
the drive is reporting "Nonrecoverable hardware error", and that
makes me think your tape drive broke and it just happened about
the time you installed the patch (a coincidence, they happen in
real life).
