Alpha 1200 / 4.0f bail during startup (stty not found)

Hi,
  its been a while since I posted a message here but the time has come
once again. I've been trying to get a machine to boot up cleanly
(installed by someone else). Its 4.0f with no patches and stock LSM to
provide some large concatenated disk. Basically as soon as the machine
tries to go multiuser it falls over with

-: stty not found

This is just after the initialisation of the 2nd cpu.

I am assumming there's an rc script somewhere which is trying to do an
stty but as /usr is not mounted at this point its failing and stopping in
single user mode.

I've check os_flags in console and they are blank and auto-action is set
to boot.

I've not seen this before and thought it maybe something from /etc/inittab
the obvious candidate maybe being the lsm stuff. I commented it out and
then discovered it does not even get to that.

Any pointers on where to look (rc scripts et al) would be welcome. I've
done a grok with grep of all the places which were obvious (/sbin/init*)
none of which seem to call stty

I've been away from alpha boxes for 3 years so please cut me some slack if
I'm missing something blindingly obvious :-)
