Problem with defragment

Defrag on my root domain is now returning (defragment -V root_domain):

Pass 1;
  Volume 1: area at block 384 ( 319616 blocks): 60% full
defragment: chk_move_file()::Can't find file set hdr - tag 2.32846 - volume 1
defragment: Error occurred during pass 1 on volume 1. Continuing...
  Domain data as of the start of this pass:
    Extents: 0
    Files w/extents: 0
    Avg exts per file w/exts: 1.00
    Aggregate I/O perf: 100%
    Free space fragments: 136
                     <100K <1M <10M >10M
      Free space: 1% 11% 38% 49%
      Fragments: 30 65 37 4

We run defrag on Saturday and Sunday nights (avoiding our backup
processes). This just started happening this weekend. On Friday, this
server (ES40 running 5.1A+pk2) crashed and rebooted itself, logging
"System panic: panic (cpu 2): kernel memory fault" (I've opened a case
with HP to look at the crash info to see what happened) - is it possible
that the root domain got corrupted somehow in the crash? If so, how do
I go about fixing it?

I don't speak for anybody but myself - that's enough trouble.
