inittab does not seem to be working

Admins,

        I have a tru64 5.1 system, pk #3. Have the following entry in the
inittab.

            telsock:3:respawn:/usr/carsplus/bin/telsock 6666

        This process died and tried to re-read the inittab file by doing an
init q and program did not respawn. Can run program from command line so
there does not seem to be an issue with the program itself. I am needing to
know what could cause this program not to respawn? I will summarize.
