[Q] vm_swap I/O error during pagein.

Three times in the last month we've had panics on our XP1000 running
4.0F (patched):

   vmunix: panic (cpu 0): k_mem_fault: IO error during kernel pagein

Normally that means a failing disk and sure enough there's a burst of disk
errors in the binary errorlog in one case just before the crash, in another
case a few hours before and in the third case the day before.

But when the box reboots 'scu verify media' doesn't report any errors and
the grown list (scu show defects) for the disk hasn't changed since the box
was delivered 2 years ago.

The disk is internal, the box is in a secure area to which only a small
number of people have physical access and the crashes have all happened at
the weekend when there's nobody around anyway. (I mention that because I
recall a case where a disk in an external storage works box gave bursts of
errors occasionally when somebody was sitting at the console).

Any suggestions?
