This is a part of the platform kit that came with your HSG/Z controller.
If you need an updated version, I would talk to your reseller.

One method would be to buy a Tru64 UNIX Solutions kit for
the appropriate storage subsystem. You might also check
the company's website. See if the StorageWorks section
is still intact and then see what free stuff is sitting
around.
