Ashraf Baker suggested running lprsetup and specifying on for 'sb'
and 'sh' to on. Doing this solved the problem.
I got several other suggestions to simply put :sh: into the
printcap, with no value. Looking at what lprsetup did to my printcap
this appears to be exactly what it did:
3.418_lw:\
<other stuff deleted>
:sb:\
:sh:
