fsck error - Solaris 2.5.1

We normally reboot and run fsck on our x86, Solaris 2.5.1 system every day.
About a week ago we ran fsck -y and it removed /usr/lib/libresolv.so.2. We
simply copied the file back and everything functioned properly. Over the
weekend, I was installing a bundle of patches with the install_cluster script.
After the install script was started, I walked away and returned while the
system was rebooting (I didn't see the error messages). As the system was
coming up, it forced me to run fsck on /usr. As the fsck was running, it had
several errors. One of the errors was to Remove the libresolv.so.2 file (I
answered No).

Another error that occurred during the fsck was in Phase 5 Check Cly Group.
The error was Summary Information Bad, Salvage? (I answered Yes). Then, it
reported CG 29: Fixed BAD Magic Number.

My questions are, Any idea why, all of a sudden we are getting these errors?
What's the fix so that we quit getting these errors? Why does fsck want to
delete the libresolv.so.2 file? We don't get the errors every time we run
fsck, but we've gotten them twice in the last week or two.
