Replacing bad disk in submirror

Hello,

     I have an Enterprise 4000 server with an external DEC storage array and one
of the disks has gone bad in the array. The disk that went bad is d9, which is
submirror 0 of mirror d11 (information gathered using metastat). I have a new
disk but wanted to know the proper procedure to replace the submirror (i.e.
reboot in single-user mode, break mirror, swap disk, etc). Looking for the
steps to ensure no data is lost and the submirror is setup properly.
