format does not see a new A1000

Hello,

I have upgraded one of our servers (e450 with 4 x StorEdge A1000 Raids)
adding 5th A1000.
Raid Manager can see the new storedge. I have upgraded the firmware to
v.03.01.04.68, created lun0 (raid5 using all disks).. format completed
successfuly. the problem is that the format command does not see the new
device. tried rebooting with boot -r, nvutil, lad and raidutil all see
the device... i also see lines in /var/adm/messages like this:

and still i can not see the device under format. could it be something
wrong in rdriver.conf ? i noticed that all my previous raids have a
different number in the "modules=" section in rdriver.conf and the new
raid has the same number as an existing raid (module=3).
 raida -> module=3
 raidb -> module=1
 raidc -> module=4
 raidd -> module=2
 raide -> module=3
could this be the problem ?

..do you have any suggestions ?
