disks un-labeling themselves.

I am having a very strange problem on a Sun 250 running Solaris 7
(Generic_106541-21). A disk is partitioned and labeled and after a random
ammount of time, appears to un-label and un-partition itself.

The drive was originally partitioned as:

Part Tag Flag Cylinders Size Blocks
  0 usr wm 0 - 884 1.00GB (885/0/0) 2097450
  1 usr wm 885 - 1769 1.00GB (885/0/0) 2097450
  2 backup wu 0 - 7498 8.47GB (7499/0/0) 17772630
  3 usr wm 1770 - 2654 1.00GB (885/0/0) 2097450
  4 usr wm 2655 - 4070 1.60GB (1416/0/0) 3355920
  5 unassigned wm 0 0 (0/0/0) 0
  6 unassigned wm 0 0 (0/0/0) 0
  7 unassigned wm 0 0 (0/0/0) 0

When it starts giving the errors, format reports the disk as unlabeled and
if I check the partitions shows:

Part Tag Flag Cylinders Size Blocks
  0 root wm 0 - 110 128.45MB (111/0/0) 263070
  1 swap wu 111 - 221 128.45MB (111/0/0) 263070
  2 backup wu 0 - 7498 8.47GB (7499/0/0) 17772630
  3 unassigned wm 0 0 (0/0/0) 0
  4 unassigned wm 0 0 (0/0/0) 0
  5 unassigned wm 0 0 (0/0/0) 0
  6 usr wm 222 - 7498 8.22GB (7277/0/0) 17246490
  7 unassigned wm 0 0 (0/0/0) 0

After this happens, if I run the verify command from the 'format>' prompt,
it returns the original paritioning info. I didn't set up this machine and
I am at a loss as to why it would be doing this. Any ideas?
