Flash and Corrupt path_to_inst and can't stat

Did a flar restore via a wanboot jumpstart server... The original image is a
Solaris 8 sun4u, going back on a sun4u.....

When it rebooted after the flash install I got:

*"**Can't stat /dev/rdsk/c0t0d0s0*

I had to rebuild my path_to_inst via boot cdrom -s....

Any reason why this happened and how can I avoid it.....
