probe-scsi-all doesn't see the disk, there's a small chance of
a bad cable,
terminator, or controller, but most likely the disk
is toast and it's time to
replace it and restore from backup (if
it contained anything besides /tmp).
Before giving up hope:
Power it all off
Re-seat the cables and terminator
Turn
it back on (if the disk in in an external enclosure, power that
up first)
