unable to book V240 machine

Hi Folks,

Actually, i was trying to recover the root password of the v240
machine. We followed the basic procedure such as boot the system using cdrom
in single user mode, then tried to mount it at temporary mount point i.e.
/tmp/mnt, while mounting the root file system we got error message as given
below:-
#mount /dev/dsk/c0t0d0s0 /tmp/mnt
/dev/dsk/c0t0d0s0:bad magic number
After that the system got rebooted and while starting up it showed following
error message:-
/dev/md/rdsk/d0: unexpected inconsistency- run fsck manually.
Warning: unable to repair / file system- run fsck manually.

and everytime it
is getting stucked at provide password to login into system maintenance mode
or press ctl D to normal startup. Also, it is not accepting password showing
incorrect login. If i press ctl+d get following messages
 
Mount: the state of
/dev/md/dsk/d0 is not okay.
         And it was attemped to be mounted
read/write.
Mount: please run fsck and try again.
 
Failed to open
/etc/coreadm.conf: read only file system.
 
Init: can not created
/var/adm/utmpx.
Init: failed write of utmpx entry:  
Init: failed write of
utmpx entry:  
Init : single user mode
 
Type control-d

Please help me out
to get rid of this problem
