Swap space disappearing

I've a production box that seems to run out of swap without warning.
I've set /tmp to mount as size=100M.

After logging on to the box, not one single command will run - they all
report 'out of memory'
A shutdown -g0 -y -i6 command yielded an output of ...
           'No swap space to grow stack for pid 1'
The box ix now stuck in a shutdown state but doing absolutely nothing.

What can I put in place to defend against these issues, and where do I
look to find out what has caused it?
I've had this issue in the past and after a reboot, dmesg reports an
'out of memory' message when the sshd tries to fork, and little else.

Any help (as always) is much appreciated.
