Multipathing issue connecting to Hitachi

I have a pair of 280R's using VCS connected to a Hitachi Tagmastore.
Each node has 2 Emulex HBA cards, connected to a McData switch for
redundant pathing. When I look at a vxdisk list <device>, it reports 2
num paths and shows 2 actual devices for that device, which is what I
would expect to see.

However, when I run a luxadm -e port, it reports only finding 1 path to
HBA ports, when I would expect to see two.

I discovered the issue after ports on the both switches where reset on
at a time for some lun mapping and my connection to the storage was
lost.

I have the latest emulex drivers and also have the latest array
libraries on the server, also, running the lputil utility shows both
hba's in READY MODE.

Would anyone know what else to look for to determine why I'm not seeing
both of my hba's.
