SF15K went totally down because of inserting a broken power supply !!

Hi,

since SUN tells me this has never happened to any of their customers
before, I want to ask your opinions/experiences about this :

- we had one of our SF15K's with 2 defect power supplies out of the 6 in
total, and called a SUN-engineer in with 2 replacement PSs

- SUN engineer removed first broken PS, inserted replacement PS, all
went fine, the SMS software detects correct activation of this new PS

- SUN engineer removed second broken PS, inserted replacement PS, and
all power of the entire cabinet shuts down !! SUN engineer removes that
replacement PS again, and the power comes back to the other PS's, SC's
restart, and start POST of all domains

Obviously this caused a small internal crisis in this company ...

My questions to you:
- has someone else had before a similar situation on a SF15K or SF25K ?
- do you regard this as technically avoidable ?
- how did you deal with this with regards to SUN support ? Did you ask
(and got) better checking of replacement parts before they are used in
such high-end servers ?
