SVM errors (bad master block on device)

Hi --

We have a system with 2 internal disks mirrored with Solaris Volume
Manager. One of the disks had gone bad (c1t1d0) and needed to be
replaced. Before removing the defective disk, I removed the database
replicas from it. [ metadb -r c1t1d0s6 ]

When I attempted to recreate those replicas [ metadb -a -c2 c1t1d0s6 ] on
the new disk, I received the following error message:

        metadb: <hostname>: has bad master block on device

I rebooted the system to see if a boot -r would fix the problem, and
noticed the following message when the system was booting :

        Unnamed device detected. Please run devfsadm && metadevadm -r to
        resolve

However, when running devfsadm && metadevadm -r the same message appears.
Has anyone seen this problem before?

Any advice on how to fix the above problem would be greatly appreciated.
