Troubles with a HBA not detecting

I've just spent most of the day trying to SAN attach a new V440.
First pass, the emulex cards (LP10000) installed just fine, and a
notification was logged in the messages announcing WWPNs and WWNNs.
However, on reboot, the 'lputil' tool failed to detect the cards any more.

After much wailing and gnashing of teeth, I've managed to determine that
the lpfc driver isn't loading 'properly'.
If I do 'devfsadm -i lpfc -v' I get:
devfsadm: driver failed to attach: lpfc

However, if I do a rem_drv lpfc
Which gives me:
Invalid Argument
Cannot remove major number binding for 272

If I then do an "add_drv lpfc" it seems to come up just fine. (Well it logs
the driver version, WWPN etc in the messages log)

At least, up until next reboot.

This is proving something of a frustration for me, so I though I'd enquire
as to whether there were further avenues of investigation that i'd not
played out yet.
