Cannot Configure port of Emulex HBA

Hi Gurus,
             I am facing with a problem. One of my customer has a SF1280
server which is connected to an EMC CX300 box through 2 HBA cards ( connected
to 33Mhz PCI slot). Now customer had purchased one more CX300 server and 1 new
HBA cards last month.At that time, I had installed the HBA on the server
(totalling to 3 HBA) and rebooted the server. When the server had reached the
OBP level , it detected three HBA successfully after which i upgraded the
firmware of the HBA.
Now, customer has purchased one more HBA for redundancy totalling it to four
HBA's connected to 4 separate PCI (33MHz) slots.
But now, when i install the fourth HBA, and reboot the server i get the
following message:-

after which both the LED's on the HBA go blank. But "prtdiag" shows me all the
HBA cards whereas in "lputil" command there is only 3 cards detected.

I tried to swap the slot also, but then too the same card gives me the
problem. Is the card gone faulty???????????????? If So why the four entries in
prtdiag???????
