Bad disk determination

Managers,

I have a sun v120 with mirrored disks that after being rebooted did not
come up. I believe that one disk had died without notice and since this
was the boot disk it was only noticed then. The issue I'm having is that
the disk is detectable but I am unable to format it. I have googled
around but have been unable to find anything useful and would like to
know if the disk is indeed dead or if I'm missing some other issue. Any
responses would be greatly appreciated.
