IBM 18 GB (DDYS-T18350) Format Problem

I recently acquired an 18GB 10K SCSI-3 drive for my Ultra-1. When I
boot Solaris 9 from the first install CD, "boot -s" at the openboot
prompt, and run format, I receive the following output:
