SunFire 280R Fatal Error (CPU0)

We have a 280R with 2 USIII+ CPUs, that has been getting the following
error:

Fatal Error reported by: (Cpu0) (PCI-PBM)
CPU0 AFSR: 2000000000000 AFAR: 7ffee002000

I have switched the CPU's, applied all patches, etc, and the problem
persists.
Has anyone seen this? How can I find out what the problem is?
