disk missing in format

Hello,

A disk failed in our D1000 (which is connected to a
e6500, and we're using veritas) and i'm having trouble
replacing it.

format is missing the entry for c2t12 and thus it is
unsuccessful when attempting to replace the disk w/
vxdiskadm. i've tried drvconfig/disks/devlinks/vxdctl
enable a few times and the disk still doesn't show up
in 'format'. is there a way to resolve this without
bringing down the server? note: there is a new disk in
the slot and the corresponding light is on.

vxdisk shows:
- - c2t12 rootdg removed was:c2t12d0s2
