SCSI reset errors

Hi All,

        I have a E3500 which suddenly started spewing out errors like on boot:

There have been changes on the machine - I swapped GBIC's on it. Only
after swapping the GBIC's, the disk started giving me these problems.
My natural inclination was to put the old GBIC's back. I did that and
now 1 out of 4 times, this disk is recognized by probe-fcal-all.
However when the system tries to access the disk, I get errors like
above.

The archives say that it could be any of these:

1) GBIC's (Can't be as they are the new GBIC's and tested to work)
2) Fiber Cable (Possible, but I doubt that other drives would work and
not this, even after swapping the drives)
3) Interface Board. (Possible, but the disk gives problems even when
its swapped. The other disks work fine)

However, if I swap the disk to a different place it still gives me
these errors and other disks in its place work fine. I'm trying to
establish what is faulty. I've come to a conclusion that it is the
disk as I can hear the head on disk trying and timing out (with
clicking and ringing noise).

Please help before I go bald.
